ACCESS is a CMS Innovation Center model designed to expand technology-supported care for people with Medicare who live with chronic conditions. HealthyConnect is proud to be a participating ACCESS provider — and there is no cost to eligible patients.

No cost to you
For eligible Medicare beneficiaries, ACCESS services are provided at no cost.
